GENDER AFFIRMING CARE
Care for adults
The Project of the Quad Cities(TPQC) and Community Health Care (CHC) have partnered together to provide gender-affirming care for individuals 18 and over. This may include:
Gender Affirming Hormone Therapy (GAHT)
Gender-affirming behavioral health services
In-person services are provided at TPQC’s clinic located at 4101 John Deere Rd. in Moline, IL. Follow-up visits can be performed in-person or via telehealth. Initial appointments can be scheduled by contacting CHC at 563-336-3000. When scheduling, please indicate you want to see provider Amanda Disch while she’s at The Project.
Individuals 18+ seeking to begin gender affirming care or those interested in transferring care from another provider are welcome.
The Project knows that gender affirming care is lifesaving care. That’s why we’re proud to serve both adults and youth at our Illinois-based clinic.
What is gender affirming care?
Gender-affirming care (GAC) is a range of healthcare services (medical, social, psychological, and behavioral) interventions that support people to live authentically as their gender identity. Every person is unique, so GAC can look different for from one person to another and change over time.

Schedule an appointment with one of TPQC’s Behavioral Health team to review the informed consent document, talk through what the process will involve, and discuss the range of typical physical and emotional effects of therapy.
Schedule an appointment with CHC provider Amanda Disch, MSN, FNP-C at TPQC. Amanda will perform a history and exam consistent with establishing care for any new patient. She will also independently review the informed consent document and discuss with the patient their desired treatment plan. Appropriate labs will be ordered as needed.
Schedule a follow up appointment with Amanda to review labs, discuss treatment options, and order therapies as appropriate. (in-person or virtual)
Prescriptions can be filled onsite at Two Rivers Pharmacy or at the pharmacy of your choice.
Follow up visits with Amanda and lab testing will occur every three months during the first year of hormone therapy or with dose changes until stable adult dosing is reached followed by clinical and laboratory testing once or twice a year once an adult maintenance dose is attained. Schedule an appointment with CHC provider Amanda Disch, MSN, FNP-C at TPQC to establish care. We ask the patient to provide records from the previous provider outlining care provided to date and the most recent lab testing and results.
Prescriptions can be filled onsite at Two Rivers Pharmacy or at the pharmacy of your choice.

Get a referral from your current Primary Care Provider (if applicable). Be sure to have a copy of your health records sent to The Project of the Quad Cities. Referrals and records can be faxed to 309-762-4481

You will have an opportunity to meet with a medical provider, a behavioral health provider, and be guided by our Youth Transgender & Gender Expansive Care Navigator throughout your entire journey. Your care team will get to know you and your parent(s) or guardian(s). This is an opportunity for all of us to talk about your goals for gender affirmation, ask questions, share information, and review your medical, social, and gender histories.
We will come up with a plan to complete a behavioral health assessment. Whether your behavioral health care is with your own counselor or with us, the goal is to learn about your experience with gender and gender dysphoria, your goals for gender affirming care, your skills and strengths, and your family and social support system as it relates to your care.
Your care team will spend time talking about both non-medical and medical gender affirming options. Discussions will cover:
Benefits and risks of any supportive medical treatment plan
Permanent and non-permanent changes related to gender affirming hormones
Fertility goals
